Jowls & the Lower Face: Your Non-Surgical Options

The lower face is one of the first places people tell me they’ve “started to notice something,” usually without being able to name exactly what. The jawline isn’t quite as clean as it was. There’s a slight heaviness at the corners of the mouth. Things feel like they’ve drifted downward a little. That softening along the jaw is what most people mean when they say jowls, and it’s one of the most common reasons people come to us with treatment planning concerns in Newcastle.

The good news is we can do a lot. The important part, and the thing that separates a good result from a disappointing one, is understanding why jowls form in the first place. If you treat the wrong thing, you can make the lower face look heavier, not lighter.

Quick answer

Jowls form because of several changes happening together: bone remodelling, downward movement, loss of the facial fat pads, and skin losing collagen and elasticity. Because the cause is layered, the best non-surgical approach treats the whole face in harmony, not just the jaw or chin. Depending on what’s driving yours, that might mean collagen stimulation with Sculptra, structural support with dermal filler placed higher in the face, skin tightening with microneedling, or a combination. And sometimes, honestly, surgery is the better answer, which we will always tell you.

Why jowls actually happen

This is the part that matters, so bear with me while I explain what’s going on underneath.

Ageing in the lower face is not just skin sagging. It’s a change across every layer, and each contributes.

The bone changes. It surprises people, but the facial skeleton remodels as we age. The jaw and the areas that support the lower face lose projection and definition, so the scaffolding the skin sits on gets smaller. Less support from beneath means more of what’s above it can descend. This can be worse if you have lost teeth or had dental work in the past, as it can change the bone shape of your lower face.

The fat pads move and deflate. The face is organised into distinct fat compartments. With age, these lose volume and, crucially, shift downward. Fat that once sat high in the cheek migrates lower, gathering along the jawline and pooling at the corners of the mouth. That accumulation, sitting on a jaw that has lost its own support, is a large part of what creates the jowl.

The skin loses collagen and elasticity. Collagen declines steadily from our twenties, and elastin, the protein that lets skin spring back, degrades too. So the skin envelope loosens and can’t hold everything in place at the same time the structure beneath it shrinks and shifts.

Put those three together, and you have jowls: descent, deflation and laxity, all at once. Which is exactly why treating only the jaw rarely works.

The mistake: chasing the jowl itself

The instinct, understandably, is to want to “fill the jowl” or put filler straight around the heavy area. But if the real problem is that the midface has deflated and descended, adding volume low down just adds weight to an already heavy lower face. It can make things look fuller and more dragged down, not lifted.

This is why assessment matters more than any single product. Before I treat anyone’s lower face, I’m looking at the proportions of the whole face, where the skin has descended from, where volume has actually been lost, and how the layers relate to each other. Often, the answer to a heavy jawline lies higher up: restoring support to the midface so the lower face is held from above rather than propped up from below.

Treating the face in harmony, not chasing the symptom, is the whole game here.

Jowls treatment in Newcastle: the non-surgical options

Once we understand what’s driving your jowls, we can match the treatment to the cause. Usually it’s a combination, sequenced sensibly.

Collagen stimulation for laxity and support. Sculptra, our collagen-stimulating treatment, stimulates your own collagen gradually over months, rebuilding structural support and improving skin quality across the lower face and the areas that hold it up. For skin that has genuinely lost its scaffolding, this addresses the actual cause rather than masking it. Polynucleotides work at a cellular level to improve skin quality and resilience, which supports thinner, laxer skin.

Structural filler, placed thoughtfully. Dermal filler for structure and definition can restore lost support, but the skill is in placement. Often that means rebuilding the midface and refining the jawline to re-establish a clean line, rather than injecting into the jowl itself. Done well, this lifts and defines. Done carelessly, it weighs the face down.

Skin tightening and quality. Medical-grade microneedling stimulates new collagen and improves the skin’s texture and firmness, helping the envelope hold better. We can pair it with exosomes, a promising add-on, though the research is still young and I will always be upfront about that; in clinic, with our patients’ results, we are bowled over by the combination approach of exosomes and SkinPen microneedling.

Which of these is right, and in what order, depends entirely on your face. That’s not a cop-out; it’s the honest answer, and it’s why a proper consultation comes before any needle.

When surgery is the better route

I want to be straight with you about the limits. Non-surgical treatments are excellent for mild to moderate jowling, and for supporting, defining and improving skin quality. But if there is significant skin laxity and substantial descent, there is a point where the most honest recommendation is surgery, such as a lower facelift, because no amount of filler or collagen stimulation will lift that much loose tissue.

If you’re at that point, I’ll tell you. I would far rather send you to an excellent surgeon than take your money for a treatment that can’t deliver what you’re hoping for. Knowing where the line is, and being honest about it, is part of good care.

The Ametrine view

Jowls aren’t a single problem with a single fix. They’re the visible result of several layers of the face changing at once, and the treatments that genuinely help are the ones chosen after really looking at your face as a whole.

Frequently asked questions

What causes jowls?

Jowls form from several age-related changes happening together: the facial bones remodel and lose projection, the fat pads deflate and shift downward, and the skin loses collagen and elasticity. It’s this combination of descent, deflation and laxity, not skin sagging alone, that creates a heavy lower face.

Can jowls be treated without surgery?

Yes, mild to moderate jowls respond well to non-surgical treatment. Depending on the cause, that might include collagen stimulation with Sculptra or polynucleotides, structural support with carefully placed dermal filler, and skin tightening with microneedling. Significant laxity, however, may need surgery.

Can filler get rid of jowls?

Filler can help, but not by injecting the jowl itself. Because jowls are often driven by loss of support higher in the face, the best results usually come from restoring the midface and refining the jawline so the lower face is supported from above. Adding volume directly into a heavy area can make it look heavier.

Is Sculptra good for jowls?

Sculptra can be very effective because it stimulates your own collagen to rebuild structural support across the lower face over several months. For jowls driven by collagen loss and skin laxity, it addresses an underlying cause rather than simply adding volume. It’s often combined with other treatments.

When do I need a facelift instead of non-surgical treatment?

When there’s significant skin laxity and substantial tissue descent, non-surgical treatments can’t lift that much loose skin, and a surgical option like a lower facelift becomes the more honest answer. At your consultation, we’ll tell you plainly if you’ve reached that point.

How do you decide what treatment I need for my jowls?

Through a full facial assessment. We look at your proportions, where skin has descended from, where volume has actually been lost, and how the layers relate, then match treatment to the cause. The aim is to treat the face in harmony rather than chase the jowl in isolation.
